Current Positions

Community Health Nurse
We are seeking a Community Health Nurse. This is an exempt, full-time position (40 hours per week) with an annual salary range of $65,000 - $75,000. This position reports to the Clinic Director. This is an ideal position for someone with a passion for our mission and experience with coordinating multiple tasks in a strong, local community organization.

About the Role
The Corner Health Center has an immediate opening for a Community Health Nurse.

Essential Duties and Responsibilities
1. Collaborate directly with clinicians to manage care of patients, particularly complex high risk patients.

2. Collaborate with the Care Manager, Social Workers, in the planning and evaluation of care.

3. Manage care management of infant and children’s services.

4. Participate with MIHP home infant services.

5. Supports the management of the VFC immunization program.

6. Oversee care management of transgender patients seeking gender-affirming care.

7. Triage phone calls via the RN Line.

8. Assist in direct patient care during clinic hours as needed.

9. Attend morning huddle and reports on pertinent information for assigned patients.

10. Maintain EHR nursing inbox and triage patient cases and lab results.

11. Oversee reporting/follow-up on all abnormal labs and tests. Notifies clinician and patient as appropriate.

12. Identify and utilize resources for referral both in the center and in the community.

13. Attend staff and clinic meetings.

14. Conducts prior authorizations.

15. Perform other duties as requested by the Clinic Director.

Required Education, Experience, and Licensure:

Patient Services Representative

About the Role
The Corner Health Center has an immediate opening for part-time Patient Services Representative. This is an hourly position, approximately 20 hours per week. This position is responsible for scheduling all patient appointments including entering and confirming information in the EMR/EHR system. Also the PSR will professionally greet and assist patients, and guests as they come into the clinic and assist in the daily operations of a busy adolescent and young adult health center.

Essential Duties and Responsibilities
• Schedule patient appointments
• Answer phones and provide telephone triage, routing calls to appropriate staff
• Ensure that returning patients’ records are reviewed to determine eligibility of service, and all patient information is updated.
• Perform check out duties, including scheduling of next appointment and collecting fees.
• Reconcile petty cash.
• Provide clinic staff with daily information regarding patient schedule as well as updates to the schedule throughout the day.
• Protect the confidentiality of member information in accordance with HIPAA laws and adhere to organizational policies regarding confidentiality.
• Attend the clinic and staff meetings.

The Ideal Candidate
• High School Diploma or equivalent.
• Experience with EMR/EHR.

HR Generalist
Position description
The Corner Health Center has an immediate opening for a part-time Human Resources Generalist for our fast-paced, team-oriented, specialized adolescent health care clinic. This is a Non-Exempt Monday through Friday position at 24 hours per week.

Core Responsibilities
• Payroll management.
• Coordinates and maintains personnel records for staff, students and volunteers.
• Coordinates hiring procedures and general orientation for new staff and interns, termination procedures for departing staff.
• Posts jobs and coordinates interview processes.
• Maintains onboarding packet materials.
• Maintains Employee Handbooks.
•Employee benefits coordination/enrollment/invoice review.
•Coordinates annual employee performance evaluation processes.
• Supports finance department .

Requirements
• Bachelor’s Degree and 2 years experience
• Strong proficiency in Microsoft Office, Adobe and other software packages
• Excellent writing and organizational skills
• Experience with ADP preferred
• Database management
• Adherence to deadlines
• Ability to complete work with discretion and minimal supervision

Licensed Clinical Social Worker
Through our E3 grant, the Corner has an established child and adolescent health center (CAHC) at Belleville High School. This center will be a mental health hub, and will serve youth 10-22 years of age, in the Van Buren school district with the support of a dedicated office manager. This is a grant funded position with potential for continuation.

Position Description
This position is currently 40 hours per week, including some evenings and early mornings, to accommodate the school schedule. As part of the Corner Health Center, the Clinical Social Worker functions as part of an integrated health model collaborating with medical providers to engage patients in supportive and behavioral health services. The candidate must like working with young people, working independently, providing evidenced based therapy, possess excellent communication skills, and working with a multi-disciplinary team.

Core Responsibilities
• Provides individual therapy to youth, young adults, and their families, with a case load of 25-27 per week.
• Conducts psycho-social evaluations, develops treatments plans, graduation plans, and safety plans as needed.
• Under supervision from the Behavioral Health Manager, this clinician coordinates and provides mental health services at the Belleville High School E3 site.
• Contributes to the overall health and well-being of the organization, and their clients, by connecting clients to other Corner Health services, as appropriate.
• Maintains consistent and thorough documentation, timely records and accurate billing.
• Maintains Employee Handbooks.
•Functions as a vital member of the Behavioral Health team.

Requirements
• State license (Michigan) LMSW, active as of the date of hire.
• Minimum 3 to 5 years clinical experience working with adolescents and young adults, preferably, diverse and underserved populations.
• Minimum 1 year experience working with families.
• Trauma-informed practice, with experience in cognitive behavioral therapy, and at least one other evidence based practice.
• Strong computer skills, including experience working with an Electronic Health Record.

Preferred Skills
• Experience preparing for grant-related audits
• Supervisory experience a plus
• Experience/service in a community health or medical setting
• Knowledge of minor consent laws and billing experience
