The Patient and Family Advisory Council is a mechanism for dialogue between patients, patient families, and medical professionals ensuring that the Corner Health Center provides exemplary patient care. Patient and families are often the most knowledgeable members of the care team, and they offer unique perspective and valuable feedback regarding the care they receive. The advisory council consist of past and present patients, family members, leaders and Corner Health Center staff members.
THE GOALS OF THE PATIENT AND FAMILY ADVISORY COUNCIL ARE:
To better the patient experience at CHC, both in terms of medical outcomes and PFM satisfaction and Identify patient and their family concerns.
To make the Corner an even more welcoming and friendly place for patients and family members.
To help medical professionals more effective reach patients and deliver care that the community wants.
Introduce any new ideas (if we have) to improve care delivery and processes.
To promote respectful, effective partnerships between patients, families and clinicians and make CHC PFMs want to remain part of the CHC community.
To help promote CHC as an important and effective community institution.
The PFAC is made up of at least ten (10) members, each who serve a one-year term on the Council with a two year option for returning members. Meetings are held three (3) times a year and the schedule is made with members at the first meeting.
