Child & Adolescent Health Center Programs

See below to learn more about the three CAHC Programs. For more detailed information, please visit MDHHS. Corner Health Center is Michigan's first CAHC site. In addition to our school-linked site, we also have a full Clinical Site (Sturgis), a School Wellness Program (WIHIMA), and an E3 Program (WAVE).

FULL CLINICAL SITE

Child and Adolescent Health Centers promote the health of children, adolescents and their families by providing important primary, preventive, and early intervention health care services. These health centers provide a range of services including, but not limited to, primary care, preventive care, comprehensive health assessment, immunizations, treatment of acute illness, co-management of chronic illness, health education and mental health services. These integrated primary care and mental health centers serve youth and adolescents ages 3-21 in schools or on school property (school-based health centers) or near schools with strong ties to those nearby schools (school-linked health centers)

School Wellness Program

The School Wellness Program (SWP) is aimed at achieving the best possible physical, intellectual, and emotional health status by providing services for children and adolescents ages 3 through 21 years. The School Wellness Program pairs a full-time RN with a mental health provider to provide a range of health and support services based on a needs assessment of the target population/community and approved by the community advisory council.  SWP’s are located on school property and provide services in no more than two school buildings.

EXPANDING, ENHANCING EMOTIONAL HEALTH

Expanding, Enhancing Emotional Health (E3) model promotes the emotional health and well-being of children, adolescents and their families by providing on-site year-round comprehensive mental health services from mild to moderate severity of need. E3 sites are located inside one school building and services are provided by a licensed master’s level mental health provider. Comprehensive services include, prevention and intervention, screening, assessing, diagnosing, treatment, education and referrals for ages 3-21.
